Important Forms available online
As noted below there are different forms for 2004 and beyond compared to 2002-3. In addition a totally different form was required prior to 2002.
If your computer has Microsoft Excel, I would recommend you use this format to track your work related injuries on the OSHA 300 log. The benefit of using Excel is the information input on the 300 log is automatically posted to the 300A log, which virtually eliminates extra work for the required annual posting of the 300A log.
FYI: All recordkeeping forms (300, 300A, 301) are required to be printed/posted on 8 1/2" X 14" paper.

PRINTING AND DOWNLOADING NON-HTML FILES:
Please note that these forms are not designed for printing
on standard 8.5 x 11" paper. If you want to print on 8.5 x 11" paper, you may
need to change your Acrobat® application settings to use "Shrink to Fit", and
you may also need to make configuration changes for your specific printer. It is
also recommended that you do not attempt to print these forms from your web
browser -- you should download the PDF file and open it directly in Acrobat®.
It is recommended that when accessing NON-HTML file links, such as Adobe
PDF, that you "RIGHT Click" on the hypertext links, and save these files
directly to your local disk. Attempting to view or print PDF files through your
browser with a plug-in viewer, can result in various technical difficulties.
Please contact your local PC support group if you have any problems downloading,
viewing, or printing these forms. If you do not have the ability to reproduce
the original 8.5 x 14" forms, you may want to contact a local printer or
publishing company to have the forms professionally reproduced for you -- simply
provide the printer with a copy of the PDF file on disk.
The Microsoft Excel template is customizable and exportable to other formats,
but the end user assumes full responsibility for making all changes, and for the
results generated by any such changes.
